Pool staff are required to:

  • Work a minimum of four (4) shifts per month
  • Work at least two (2) weekend shifts per month
  • Work at least two (2) weekday shifts per month
  • Work a minimum of two (2) recognized holidays per calendar year

St. Camillus is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Skilled Nursing Facility team. In collaboration with Registered Nurses and an interdisciplinary healthcare team, the Staff LPN provides high-quality nursing care that supports the physical, emotional, and spiritual well-being of our residents.

Position Summary

The Staff LPN utilizes the nursing process and SBAR (Situation, Background, Assessment, Recommendation) communication techniques to provide exceptional resident-centered care. This role is responsible for medication administration, treatments, clinical documentation, resident monitoring, and supervision of Certified Nursing Assistants while helping residents achieve their highest level of health and independence.

Key Responsibilities
  • Administer medications and treatments safely and accurately.
  • Monitor resident health status and report changes to the RN and healthcare team.
  • Collaborate with RNs in assessing, implementing, and evaluating resident care plans.
  • Complete timely and accurate charting, daily documentation, and assessments.
  • Coordinate physician orders, appointments, laboratory services, and transportation arrangements.
  • Provide leadership, guidance, and oversight to CNAs on the unit.
  • Respond effectively to emergency situations to ensure resident safety and well-being.
  • Participate in care planning, quality improvement initiatives, and interdisciplinary team meetings.
  • Maintain compliance with HIPAA, infection prevention, safety, and regulatory requirements.
  • Communicate effectively with residents, families, physicians, and team members to promote continuity of care.
Qualifications
  • Graduate of an accredited Practical Nursing program.
  • Current, unrestricted Wisconsin LPN license.
  • Current CPR certification.
  • Strong verbal and written communication skills.
  • Knowledge of long-term care regulations, including OBRA, HSS 132, and Wisconsin Nurse Practice standards.
  • Experience with electronic health records and accurate data entry.
  • Excellent organizational skills with the ability to prioritize and manage multiple responsibilities.
  • Ability to safely perform essential job functions with or without reasonable accommodation.
