The Role:
Ideal candidates are those who thrive working with a collaborative and compassionate community of caregivers. For this position, Nurse Practitioners will manage patients in an outpatient clinic setting as well as providing in-home clinical support for patients in the community. GGH Nurse Practitioners will be supporting clients with their patient panels and partnering with clinics as an embedded member of the care team with the goal of preventing unnecessary acute care admission, providing holistic patient care, optimizing the management of the patients' medical conditions, and providing support for clinical and psychosocial factors that contribute to the cyclical utilization of healthcare.
What will you do as Nurse Practitioner at GGH?
  • Assess patients' clinical problems and healthcare needs and provide clinical care and consultation
  • Perform an annual wellness exam for senior Medicare patients in a designated clinic or at their place of residence. Obtain and record the patient's medical history and symptoms, including medicine reconciliation
  • HCC coding, verification, and risk adjustment
  • Coordinates continuity of care, prevention and avoidance of complications, and close clinical treatment and management under the direction of the patient's primary healthcare provider(s) and collaboration with other referral sources
  • Perform various tests and screenings
  • Counseling patients with education around their discharge plan
  • Execute the discharge plan and use your critical thinking skills to build an effective care plan during a critical transition period after a hospital discharge
  • Supports patients and their families, and provides compassionate care
  • Contribute to an interdisciplinary team (IDT) model and attend regular IDT meetings to collaborate with other healthcare professionals (social workers, case managers, and other clinicians) to ensure the holistic approach to the care plan is carried out
  • Collaborate with care team to ensure completion of holistic patient care
  • Ability to travel within the Medford, OR area 5 days a week to provide care to patients in their home based on the needs of business/market
  • Expected patient volume completed per day in clinic: 8-10
  • Expected patient volume completed per day in-home: 5-6
  • Complete charts within 72 hours of patients' visits; you will have access to EMR and/or portals to review charts and document notes
  • Cover for other Nurse Practitioners and Care Team Members when necessary
  • Maintains patients' confidence by completing and safeguarding medical records, keeping patients' information confidential, and abiding by all HIPAA rules and regulations

This position is a full time Nurse Practitioner opportunity (5 days a week, 8am-5pm schedule). This role can include in clinic, in-home visits or telehealth visits. Home visits will be scheduled within a 30 mile radius of your home region (Jackson County, OR).

Experience and qualifications
  • Current Oregon Nurse Practitioner license
  • 2 + years of clinical experience as a NP in either Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
  • Acute care experience with high-risk patients is a plus
  • Home health experience (NP or RN) is a plus
  • Nurse Practitioner certification from either the ANCC / AANP
  • Working knowledge of chronic care management and various medication options
  • Current DEA licensure / Prescriptive Authority post-hire
  • Active CPR/BLS Certification
  • Current Oregon Driver's License
