Healthcare Sales | Clinical Liaison | Business Development | Skilled Nursing

Champion Care is seeking an experienced Hospital Liaison to support our skilled nursing and rehabilitation centers throughout the Madison, Beloit, Janesville, and Southern Wisconsin market.

This is an excellent opportunity for an experienced Hospital Liaison, Clinical Liaison, Healthcare Sales Representative, Admissions Liaison, Community Liaison, or Healthcare Business Development professional who understands hospital referrals, discharge planning, skilled nursing, rehabilitation, and post-acute care.

The Hospital Liaison will develop relationships with hospital case managers, social workers, discharge planners, physicians, healthcare providers, and community referral sources to increase appropriate referrals and admissions to our skilled nursing facilities.

Hospital Liaison Responsibilities
  • Develop and maintain strong relationships with hospitals, case managers, social workers, discharge planners, physicians, and healthcare referral sources

  • Generate new skilled nursing and rehabilitation referrals throughout Madison, Beloit, Janesville, and surrounding Southern Wisconsin communities

  • Serve as a key clinical and hospital liaison between referral partners and our skilled nursing facilities

  • Conduct regular hospital visits and build relationships with hospital discharge planning and case management teams

  • Educate referral sources on our skilled nursing, short-term rehabilitation, nursing care, and post-acute services

  • Identify new opportunities for healthcare business development, hospital partnerships, referrals, and census growth

  • Maintain consistent communication with hospital case management and discharge planning teams

  • Work closely with facility Administrators, Directors of Nursing, Admissions Directors, and regional leadership

  • Assist with the referral and admissions process to help remove barriers and facilitate timely patient transitions

  • Develop relationships with physicians, clinics, healthcare organizations, senior care providers, and other community partners

  • Monitor referral activity, healthcare market trends, competitor activity, and business development opportunities

  • Represent Champion Care professionally throughout the Madison and Southern Wisconsin healthcare community

Qualifications
  • Previous experience as a Hospital Liaison, Clinical Liaison, Admissions Liaison, Community Liaison, Healthcare Sales Representative, Account Executive, or Business Development Representative preferred

  • Experience in skilled nursing, post-acute care, rehabilitation, hospitals, home health, hospice, senior care, or healthcare sales strongly preferred

  • Knowledge of hospital discharge planning, case management, patient referrals, admissions, and post-acute placement

  • Existing healthcare relationships in Madison, Beloit, Janesville, or Southern Wisconsin are highly desirable

  • Strong sales, networking, communication, and relationship-building skills

  • Ability to develop new referral sources while strengthening existing hospital relationships

  • Self-motivated, organized, and comfortable working independently in a field-based healthcare sales position

  • Ability to collaborate effectively with clinical, admissions, nursing, and operational leadership
