Job description

Position Summary: To provide quality care to clients in the client's home for crisis and urgent need coverage situations as needed in locations within the four boroughs as specified by the plan of care and in accordance to established policies, procedures.
Principal Responsibilities:
  • Recognizes and knowledgeable about essential job functions/task with the HHA scope of practice.
  • Performs and documents service delivery as specified on care plan.
  • Participates in visit verifications and supervisory/quality field visits as assigned.
  • Employs principles of Infection Control and Universal Blood and Body Fluid Precautions.
  • Complies with agency confidentiality policies, and maintains respect for client privacy.
  • Attends annual mandatory in-services
  • Adheres to organization's standards by completing annual mandatory trainings in a timely manner on topics including - but not limited to - Sexual Harassment Prevention Training, IT Security Awareness, Compliance, OSHA, HIPAA and any other training the organization may deem necessary in regards to regulatory compliance or good business practices.
  • Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
  • Adheres to the organization's policy in regards to absenteeism and appearance.

Salary Range: $20 - $21 per hour commensurate with experience
Job Competencies & Minimum Qualifications:
  • HS Diploma/GED preferred; experience accepted as substitute
  • NYS DOH Home Health Aide Certified
  • 2-4 years of experience as a home health aide
  • Experience with elder care and techniques for dementia and Alzheimer client care
  • Excellent reporting skills
  • Excellent verbal and written communication skills
  • Excellent customer service and organizational skills

Working Conditions/Physical Demand:
  • Occasional exposure to outdoor weather conditions, moving mechanical parts, and occasional risk of exposure to pathogens.
  • While performing the duties of this job, the employee is occasionally exposed to outdoor weather conditions, risk of exposure to pathogens and unsanitary conditions in client's homes.
  • Must be able to travel locally by public transportation to make home visits. Must have valid driver's license, car registration, and insurance if applicable to the position.
