On Call Hospice RN-Nights

Location: Bergen County

We are seeking a compassionate and dedicated On-Call Hospice Registered Nurse (RN) to join our team. This role is crucial in providing after-hours care and support to hospice patients and their families. The ideal candidate will respond to patient needs during night shifts, ensuring high-quality, end-of-life care in accordance with the hospice philosophy.

Responsibilities:

  • Patient Care: Provide nursing care to hospice patients during after-hours, addressing physical, emotional, and spiritual needs.

  • Emergency Response: Respond promptly to patient calls, perform on-site visits as needed, and manage urgent patient needs.

  • Symptom Management: Assess and manage pain and other symptoms, providing comfort and relief to patients.

  • Family Support: Offer guidance and emotional support to families during challenging times.

  • Collaboration: Communicate effectively with the interdisciplinary hospice team, including physicians, social workers, and chaplains.

  • Documentation: Maintain accurate and timely records of patient care and communication.

  • Adhere to Best Practices: Follow VNA Health Group's hospice best practices, ensuring compassionate and patient-centered care, effective communication, and continuous quality improvement.

Qualifications:

candidates must:

  • Education: Current NJ RN license

  • Experience: Minimum of 1-2 years of hospice or palliative care experience preferred.

  • Skills: Strong assessment, critical thinking, and communication skills.

  • Availability: Ability to work night shifts, including weekends and holidays as scheduled.

  • Transportation: Reliable vehicle, valid driverโ€™s license, and proof of auto insurance.

Working Conditions/Physical Demand: Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $89,700 to USD $97,800.
