Job description

What you should know about the RN Hospice House/IPU role:

  • This is a Full-time 6:30am-7pm role with every 3rd weekend rotation at our inpatient unit located at Ohio State University Wexner Memorial Center.
  • Collaboration with colleagues required.
  • We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed

The RN Hospice House/IPU Duties are:

  • Plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing.
  • Assess and evaluates the patient and family including the physical, spiritual, and psychosocial needs.
  • Develops and implements an individualized plan of care including creating measurable patient-centered goals in collaboration with the patient and family, that consistently is updated and reviewed to reflect assessment findings to ensure quality patient care.
  • Reviews and updates plan of care including interventions, medication profile, visit frequency to meet the patient’s changing needs.
  • Assess and reports to physician changes of patient condition, level of care needs, and facilitate determining eligibility for services.
  • Collaborates and participates as member of hospice team in regularly scheduled care conferences, hospice admissions, and Interdisciplinary Team (IDT) meetings, to ensure coordination of services.
  • Documents physicians’ orders when received in electronic health record to provide accurate information and ensure safe delivery of services.
  • Supervises, teaches and evaluates care provided by Personal Care Specialists, Volunteers and other care givers to ensure compliance to the plan of care.
  • Documents assessments, interventions and all changes thoroughly at the point of service in the electronic health record and synchronizes this documentation for each patient according to Ohio’s Hospice standards to ensure quality communication with patient’s Interdisciplinary Team.
  • Communicates handoffs such as SBAR assessments and other methods, timely to ensure communication is relayed appropriately for continuation of quality care.
  • Assists in other activities and departments when requested to ensure complete and thorough patient care for all Ohio’s Hospice patients.
  • Participates in Continuous Quality Improvement activities to support the quality of Ohio’s Hospice services.
  • Precepts new staff to support professional relationships with newly hired team members as required.

Qualifications:

  • Graduate of an accredited RN college; Bachelor of Science in Nursing (BSN) preferred
  • Active Registered Nurse license in the State of Ohio without any board actions.
  • Two years acute care nursing experience preferred
  • Certification in Hospice & Palliative Medicine (CHPN) preferred and will be made available to Ohio’s Hospice employees who wish to obtain certification.
  • CPR Certified
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime or inclement weather.
  • Valid Driver's License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage
