Position Summary

The Case Manager is responsible for supporting residents through comprehensive case management services, helping them navigate legal, personal, educational, employment, and community resource-related needs. This role serves as a key advocate for residents by coordinating care, maintaining compliance requirements, connecting residents to appropriate resources, and developing individualized plans that support long-term success and recovery.

Key Responsibilities

Case Management amp; Resident Support
  • Schedule and conduct meetings with residents to assess needs and track progress.

  • Develop individualized treatment plans designed to support residents' personal, legal, educational, and employment goals.

  • Work collaboratively with care teams to ensure residents receive appropriate support and services.

  • Maintain accurate and timely case notes, documentation, and resident records.

  • Monitor resident progress and identify barriers to success.

  • Assist residents in accessing community resources, benefits, and support services.

  • Provide guidance and accountability to residents throughout their recovery journey.

Legal Case Management
  • Serve as a liaison between residents and external agencies, including probation officers, courts, attorneys, and other legal entities.

  • Prepare and submit monthly program update letters and other required documentation.

  • Represent No Longer Bound in court proceedings when necessary.

  • Maintain detailed records regarding resident legal matters and case status updates.
