Overview
- Assess the counseling needs of the client including appropriate level of care placement
- Assess and diagnose client for mental health/ substance use diagnoses using DSM and/or ASAM criteria
- Incorporate the concepts of evidence-based practices relating to recovery, trauma-informed care, CBT, DBT, etc.
- Be familiar with all State and Federal (OMHSAS, DDAP, SAMHSA) regulations for the Program
- Improve access to high-quality care for individuals across the life span
- Promote integration of behavioral health, drug and alcohol, and physical health care services using Evidence-Based Practices (EBPs) in service provision
- Provide individual, group, family, and IOP counseling
- Build and enhance the relationship with program staff and outside providers to coordinate care
- Complete all clinical documentation required, intake assessments for open access service
- Obtain previous records that are relevant to the current treatment episode
- Prepare individualized treatment plan(s) and provide mental health counseling and co-occurring portions of the client treatment
- Provide coordination of support services for the client/family and record the services in the clients clinical record following HIPAA laws and regulations
- Contact and coordinate with referral sources, (i.e. legal, school, employer, etc.)
- Facilitate needed interventions for crisis-oriented issues utilizing a multi-disciplinary team approach for assigned clients
- Contribute to practice on related tasks as needed
Responsibilities
- Clinical counseling: 2 years (Required)
- Experience working with adults with SUD & Mental Health Diagnosis
- Experience working in an Outpatient setting preferred
- Experience with trauma-informed service, cognitive-behavioral therapies including DBT, and motivational therapies including the use of incentives preferred
- Strong interpersonal and engagement skills
- Ability to work independently and in collaboration with multidisciplinary team
Qualifications
- Licensed Clinical Social Worker (LCSW) in Delaware or
- Licensed Professional Counselor in Mental Health (LPCMH) in Delaware
