Job description

Day-to-Day Responsibilities

The Integrated Duals Program Coordinator II supports care coordination and member engagement activities while serving as an advanced resource across multiple teams. This role ensures timely, effective, and member-centered services through documentation oversight, care coordination support, escalation management, and mentorship of Program Coordinator I staff. The position plays a key role in promoting continuity of care and operational efficiency across the care coordination team.

Key Responsibilities:

  • Perform all responsibilities of the Integrated Duals Program Coordinator I, including member and provider outreach, coordinating services and schedules, documenting care coordination activities, and ensuring compliance with contractual and regulatory requirements.
  • Serve as the first point of escalation for complex or urgent member and provider issues, coordinating responses and partnering with supervisors and cross-functional teams to ensure timely resolution.
  • Mentor and support Program Coordinator I staff and new team members by answering questions, sharing best practices, and ensuring consistency in workflows and documentation.
  • Maintain accurate and timely documentation across internal systems to support visibility, continuity of care, and collaboration with clinical teams.
  • Manage complex and urgent escalations through warm transfers, real-time issue resolution, and coordination of downstream support as needed.
  • Coordinate provider and member scheduling, support high-volume outreach initiatives, and ensure tasks are completed across multiple systems.
  • Proactively engage members through outreach efforts, including phone calls, information gathering, and follow-up activities.
  • Identify social support and community-based needs and facilitate non-clinical referrals in partnership with care managers.
  • Monitor service delivery to ensure appropriate care and compliance with waiver and program requirements, including the most complex cases.
  • Maintain accurate documentation of member contacts, case notes, referrals, and assessments in accordance with accreditation, compliance, and organizational standards.
  • Document care coordination activities and member interactions in a timely manner, following CareSource policies and professional documentation standards.
  • Identify opportunities for process improvement that enhance the member experience and share recommendations with leadership to develop scalable best practices.
