While the summer months are often considered slower in Phoenix, our outpatient clinic and inpatient SNF continue to experience strong demand - we are fully booked with an active patient waitlist. Enjoy flexible scheduling; achieve work-life balance on your terms, we are open Monday-Sunday and offer flexible 5-day work schedule week. (Example: Sun-Thurs, Mon-Fri, or Tues-Sat). While working one weekend day is preferred, we strive to accommodate your needs!

Pay Rate: $40 - $50 per hour

As a Physical Therapist, you'll provide exceptional services within Assisted Living, Skilled Nursing, and Memory Care residents. This role will foster a positive, supportive environment for residents, families, and team members on a PRN basis.

What you'll do:

  1. Responsible for the maintenance and accuracy of records in regard to patient care for all patients on personal case load.
  2. Responsible for the correct reporting of billing information of Occupational Therapy in regard to all patients on personal case load.
  3. Provide direct patient care as prescribed, including evaluation and treatment planning.
  4. Responsible for the reporting and interpretation of treatment results.
  5. Participate in development and presentation of in-services for the facility as requested.
  6. Supervise support personnel in immediate work areas as appropriate.
  7. Perform clinical functions: screens, evaluations, development of treatment plans, and delivery of treatment in accordance with facility, Medicare, and Acacia Health Centers policies and procedures. 8. Participate in meetings regarding patient care as appropriate.
  8. Assist when appropriate with any community education and public relations activities of the facility.
  9. Maintains a high standard of resident care.
  10. Responds immediately to all crisis and/or calls for assistance.
  11. Actively participates in in-services, department meetings and continuing education.
  12. Provide education to patient's family as well as staff on the specific treatment plan as well as therapy goals for the patient.
  13. Responsible for the coordination and cooperation with other staff members in carrying out the treatment plan as established.
  14. Maintains a clean and safe environment for residents, visitors and co-workers.

Who you are:

An Occupational Therapist professional with a minimum of a Bachelor's Degree in Occupational Therapy. Sedentary tasks related to job – approximately 10% Physical tasks as listed – approximately 90%. CPR certified. Licensed by the State in the designated geographic area in their respective therapy profession.
