Summary

Position Summary:

The Care Navigator will connect with patients in the practice. The primary focus of the role will be to coordinate care, manage transitions, quality measures, and support patients with complex needs, often focusing on population health management and improving outcomes within an integrated care team model. Working closely within the clinic to cover Lifestyle Health and other areas that may need nursing support.

Position Qualifications:

Minimum Education:

Associate's or Diploma degree from a registered nurse program

Bachelor's degree in nursing preferred

Minimum Experience:

Six months experience in Healthcare

Required Registration/License/Certification:

Current Kansas Registered Nurse license
