Job Description: RN Care Manager Care Management Team
Location: Santa Monica, CA
Schedule: Full-Time, 8-hour Day Shifts (including weekend rotations as needed)

Overview:
The RN Care Manager is a critical member of the Care Management Team, collaborating with patients, caregivers, physicians, social workers, payers, and other healthcare professionals. This role is responsible for promoting effective communication, transitional care, and utilization management, all while respecting patient autonomy and addressing social determinants of health.

Responsibilities:

  • Coordinate and facilitate patient care across the healthcare continuum
  • Work closely with interdisciplinary teams to ensure appropriate resource utilization
  • Support patients through transitions of care to ensure continuity and quality outcomes
  • Participate in weekend rotations and contribute to a team-oriented environment

Required Qualifications:

  • Bachelor's Degree in Nursing or equivalent experience
  • Current California Registered Nurse License
  • Minimum 2 years' experience in a healthcare-related field (acute, ambulatory, post-acute, etc.)
  • Minimum 2 years' experience in Case Management (Care Coordination or Utilization Management), or successful completion of the Care Manager Transitions in Practice (TIP) program

Preferred Qualifications:

  • Master's Degree in Nursing or a healthcare-related field
  • Certification in Case Management (CCM) or Accredited Case Manager (ACM)

Compensation & Benefits:

  • Base Salary: $117,000 $182,000
  • Comprehensive benefits package, including medical, dental, vision, retirement plans, and paid leave
  • No relocation assistance or bonus eligibility

Candidate Requirements:

  • 2+ years of relevant experience
  • Bachelor's Degree in Nursing
