Jethro Health System is looking for an experience Home Health Agency RN to see patients in the North Shore area: Lynn, Peabody, Nahant, Revere, Winthrop, East Boston, Chelsea, Salem, Swampscott, Wakefield, Reading, Woburn, Saugus, Revere.

We are seeking a compassionate and dedicated RN to join our Home Health team. The ideal candidate will provide quality care to patients in the comfort of their homes in the Lynn area, ensuring their well-being and promoting independence.

This is a part-time position, with potential to become Full-Time.

Responsibilities

  • Conduct thorough assessments of patients' health status and needs in their home environment.

  • Develop and implement individualized care plans based on patient assessments and medical history.

  • Provide education and support to patients and their families regarding health conditions, medications, and self-care techniques.

  • Assist with discharge planning from hospitals or rehabilitation facilities, ensuring a smooth transition back to home care.

  • Monitor patient progress and adjust care plans as necessary to meet evolving health needs.

  • Collaborate with interdisciplinary teams, including physicians, social workers, and therapists, to coordinate comprehensive care.

  • Document all patient interactions accurately in compliance with ICD-10 coding standards.

  • Offer health coaching to empower patients in managing chronic conditions effectively.

  • Provide specialized services such as esthetic laser treatment when applicable.

  • Support family planning initiatives and educate patients on related health topics.
