Qualifications:
• Current licensure, or eligibility for licensure, as a Physical Therapist in MA.
• Graduate from an accredited physical therapy program.
• Stellar communication skills, paired with a genuine, patient-centered approach.
• Capability to proficiently manage a varied caseload.


Responsibilities

  • In this pivotal role, your expertise will guide one-on-one therapy services to patients in their homes, crafting a significant impact on their recovery and well-being

  • Duties will span from assessing patient needs, sculpting individualized treatment plans, conducting therapy sessions, and vigilantly tracking patient progress

  • Therapist Autonomy: We entrust our therapists with the autonomy to make impactful decisions regarding patient care, valuing your expertise and insight in crafting patient treatment plans

  • Conduct thorough in-home patient evaluations

  • Formulate and enact effective treatment plans, distinctly tailored to each patient’s needs and objectives

  • Educate and continuously engage with patients and their families about home exercise programs and other pertinent care details

  • Collaborate with our adept team of healthcare professionals, ensuring each patient receives premium, holistic care

  • Create and implement personalized care plans

  • Collaborate with team members to ensure quality outcomes

  • Maintain documentation to meet all regulatory requirements

  • Ensure all required physician orders are obtained and up to date
