• Conduct thorough and empathetic virtual comprehensive psychosocial assessments to evaluate clients' strengths, challenges, and needs, ensuring tailored treatment approaches.
• Provide evidence-based individual, family, and group therapy sessions to assist clients in understanding and managing their mental health issues.
• Collaborate with interdisciplinary healthcare teams, which may include nurses, doctors, and other social workers, to coordinate integrated care strategies that align with clients' overall wellness goals.
• Advocate assertively for clients' rights, ensuring they have access to appropriate community resources, support services, and necessary entitlements during their hospice care.
• Maintain meticulous documentation of client interactions, therapeutic interventions, treatment progress, and outcomes in line with electronic health record (EHR) best practices.
• Participate actively in team meetings, case conferences, and ongoing continuing education to enhance your clinical skills and professional development.

Your primary focus will be on clients with serious, life-limiting diagnoses receiving hospice care. These individuals may be experiencing significant psychological distress, grief, or anxiety about their conditions. You will work closely not only with these patients but also with their families, providing necessary guidance and emotional support during challenging times.

• A Master's degree in Social Work from an accredited institution is required.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in Arizona.
• A minimum of three years of post-licensure experience in providing clinical social work services, with a particular focus on mental health.
• Expertise in conducting diagnostic assessments and developing effective treatment plans tailored to individual needs.
• Strong interpersonal and communication skills, enabling you to build rapport with diverse populations, including various age groups and cultural backgrounds.
