Opportunity:

RN - Home Health Nurse

Job Details:

• 13-week contract starting ASAP
• 36 guaranteed hours per week
• 8-10 hour day shifts
• Every 3rd weekend and holiday rotation required - weekend coverage subject to change based on nursing pool needs
• Every other holiday rotation required
• On-call every 4th weekend and 1 day during the week
• Average of 5 patient visits per day with potential for up to 6 visits depending on territory, patient census, and staff availability
• Travel up to a maximum of 150 miles in a given day (varies by day)
• Perform Oasis admissions, all time points, and episode management
• Responsibilities include physician calls, coordination of care, and timely documentation in EMR
• Clinical competencies required in wound care, IV, ports, lab draws, and chronic disease management
• Oasis charting experience required; Epic preferred
• Minimum 2+ years of home health experience preferred
• Required certifications: AHA BLS
• Active Michigan and Wisconsin nursing license required
• Valid driver's license required
• Associate Degree or Certificate required; Bachelor's Degree preferred
