  • JOB RELATIONSHIPS
    • Responsible to the Agency Director. Consults with members of the Home Health team as appropriate.
  • DEFINITION OF POSITION
    • Lead with Heart, Manage with Precision: Clinical Operations Manager
      We are seeking a dynamic RN Leader to oversee the daily clinical heartbeat of our Home Health and Hospice nursing and CNA teams. As our Clinical Operations Manager, you won't just manage charts; you'll mentor a team of clinicians to operate at the top of their licenses while ensuring our patients receive the gold standard of coordinated care.
    • What You'll Do:
      • Drive Clinical Excellence: Oversee the intake/referral process, facilitate interdisciplinary meetings, and ensure every care plan is individualized and effective.
      • Empower the Team: Lead staff orientation, provide real-time coaching, and conduct ride-longs/rounding to ensure our clinicians feel supported and heard.
      • Champion Quality & Safety: Facilitate our QAPI program, monitor infection control, and stay ahead of Medicare/Medicaid regulatory updates.
      • Bridge the Gap: Step into the field or assist with patient care coordination when the team needs an extra hand-leading by example is part of our DNA.

    • Responsible for the continuous improvement in assessing, developing, and delivering a well-organized plan of care as it relates to nursing services and case management.
    • Supervises coordination of services and care for clients accepted for Home Health and Hospice and ensures the plan of care is developed, implemented, and updated.
    • Responsible for management and development of clinical staff.
    • Responsible for working within the scope of the Nurse Practice Act regarding judgment, nursing skills, education, and experience.
    • Clinical Operations Manager is available during all operating hours for staff and patient needs.
  • QUALIFICATIONS
    • Professional
      • Current license to practice as an RN in the state of North Carolina.
      • Knowledge of current trends and techniques in nursing appropriate to practice setting.
      • Experience working as an RN within the last three years, with at least two years clinical experience in Medical/Surgical Nursing or Public Health/Home Health/Hospice setting preferred.
      • Knowledge of coverage issues and reimbursement regulations affecting Home Health preferred.
      • Current BLS Healthcare Provider Certification
      • Maintain a valid North Carolina Driver's License, auto liability insurance, and dependable transportation.
