Home Health & Hospice Registered Nurse (RN)

Location: Concord, NH (Serving Concord, Tilton, Meredith, and surrounding communities)

Pay: $37.00–$50.00/hour (based on experience)

Schedule: Full-Time | Flexible scheduling available, including a 4x8-hour schedule | On-call one night per week | One weekend per month

Home Health & Hospice RN – Build Meaningful Relationships with Patients

We're seeking a compassionate Home Health & Hospice Registered Nurse (RN) to join our growing team. This unique role offers approximately a 50/50 split between Home Health and Hospice, allowing you to care for patients throughout the continuum of care—from recovery at home to compassionate end-of-life support.

Whether you have experience in Home Health, Hospice, Med-Surg, Emergency Department, ICU, Telemetry, or another acute care setting, we'd love to hear from you. If you have at least one year of RN experience, we'll provide the training and support needed to succeed in home-based care.

You'll serve patients throughout the Concord, Tilton, and Meredith communities while working alongside an experienced interdisciplinary team dedicated to delivering exceptional, patient-centered care.

Responsibilities
  • Provide skilled nursing care to Home Health and Hospice patients in their homes.

  • Complete comprehensive nursing assessments and develop individualized care plans.

  • Manage an average of 5 Home Health visits or 4 Hospice visits per day.

  • Monitor patient progress and adjust care plans as needed.

  • Provide pain and symptom management for Hospice patients.

  • Educate patients and caregivers on disease management, medications, and treatment plans.

  • Coordinate care with physicians, therapists, social workers, chaplains, and other members of the interdisciplinary team.

  • Document patient care accurately using KanTime EMR.

  • Participate in interdisciplinary team meetings and quality improvement initiatives.

Qualifications
  • Active Registered Nurse (RN) license in the State of New Hampshire.

  • Minimum of 1 year of RN experience required.

  • Home Health and/or Hospice experience preferred.

  • Candidates with Med-Surg, ER, ICU, Telemetry, Step-Down, Progressive Care, Skilled Nursing, Rehabilitation, or other acute care experience are encouraged to apply.

  • Paid training provided for experienced RNs transitioning into Home Health or Hospice.

  • New graduates are not eligible.

  • Valid driver's license and reliable transportation.

  • Strong assessment, communication, and documentation skills.

  • KanTime experience is a plus but not required.
