• Engage collaboratively with healthcare team members to assess and address a diverse range of client needs related to mental health and psychosocial support.
• Develop, implement, and monitor personalized care plans tailored to the individual circumstances of each patient and family.
• Provide education to clients and their families about mental health services, resources, and coping strategies relevant to their specific conditions.
• Utilize evidence-based practices and continuously update your knowledge of the latest therapeutic techniques to enhance patient care.
• Document all interactions and interventions clearly and accurately in patient records to ensure compliance with regulatory standards and quality assurance.

This role will focus primarily on working with senior populations facing critical illness and end-of-life concerns. You will provide compassionate support and expertise to patients struggling with complex emotional and psychological challenges, ensuring their dignity and comfort during a crucial phase of life. Engaging with family members is also a core aspect of your role, as you will help navigate the emotional landscape of grief and loss.

• A Master’s degree in Social Work from an accredited institution.
• Possession of a valid Licensed Clinical Social Worker (LCSW) license.
• Excellent clinical assessment skills and the ability to communicate effectively with patients, families, and team members.
• Strong ethical framework and a commitment to confidentiality and professional standards in social work practice.

• At least 2 years of experience working in mental health settings, particularly with hospice or palliative care populations.
• Familiarity with telehealth platforms and electronic health record (EHR) management systems.
• Experience in providing training or supervision to junior staff or interns may be beneficial.
