Job description

HCH is seeking an experienced and compassionate Family Nurse Practitioner to provide comprehensive primary care to patients of all ages. The ideal candidate must be highly skilled in IV insertion, infusion therapy, and administering injections. This role focuses on ensuring continuity of care, patient safety, and clinical excellence.ย 
Responsibilities
- Provide primary and preventive care, including diagnosis, treatment, and follow-up.ย 
- Perform IV insertions, manage infusions, and administer injections safely.ย 
- Conduct physical exams, interpret labs, and develop treatment plans.ย 
- Manage acute and chronic conditions across the lifespan.ย 
- Maintain accurate documentation in the EHR system.ย 
- Collaborate with physicians and staff to ensure coordinated, high-quality care.ย 
Qualifications
- Masterโ€™s or Doctorate in Nursing; licensed FNP (FNP-C or FNP-BC) in Texas.ย 
- Minimum 2 years of experience in family or primary care preferred.ย 
- Proficient in IV therapy, venipuncture, and injection administration.ย 
- Current BLS and ACLS certifications.ย 
- Strong communication, clinical judgment, and teamwork skills.ย 
Preferred Skills
- Experience with infusions.ย 
- Familiarity with telehealth and EHR systems.ย
