What You Will Do

  • Provide care across multiple patient homes within designated service areas or regions

  • Incorporate anddemonstratestandard elements of professional practice: Assess, Outcomes Identification/Solutions, Plan, Implement, Evaluate

  • Act independently within license, scope of knowledge, and experience

  • Retain accountability for delegation, decisions, and outcomes

  • Collaborate with key stakeholders and contribute to quality improvement

  • Exhibit agility and willingness to take on new responsibilities

  • Embracenew ideasand cultural differences while managing competing priorities

Minimum Qualifications

Current Registered Nurse license in the state of practice (no flags or restrictions)

Current BLS Certification from the American Heart Association

Minimum of two years of acute care nursing experience

Current Driver's License, reliable transportation, and automobile insurance

Home care experience preferred

Preferred Clinical Skills & Experience

  • Foley and Supra Pubic Catheters - insertion, maintenance, troubleshooting

  • PleurexCatheters - drainage and site care

  • Wound Care - including Wound V.A.C. therapy

  • PICC Line Dressing Changes - sterile technique and site monitoring

  • G-Tube Management - feeding, flushing, site care

  • Medical Management - general patient care and chronic condition support

  • IV Insertion and Administration

  • Lab Draws - venipuncture and specimen handling
