Overview
Registered Nurse - Home Health (Travel) position in Battle Creek, Michigan, starting 06/22/2026 for 13 weeks. Weekly pay: 1,702-1,817 per week, guaranteed hours: 40 per week. Shift: Weekend field nurse (Saturday and Sunday) with 16-hour days, 7:00 am-11:00 pm. Setting: continuing care retirement community (CCRC) environment. This is a travel temporary assignment managed through Healthforce Healthcare LLC.

Responsibilities
- Perform in-person participant assessments to determine appropriate levels of care per the plan of care, triage urgent needs, and respond to communications from PCPs and support staff.
- Conduct regular field visits to provide nursing care, including wound care, IV hydration, lab draws, and comprehensive health assessments.
- Document interactions and visits in the EMR; initiate emergency treatment as needed; update orders and PUIs as indicated.
- Educate participants, DPOA, family, and facility staff; provide ongoing instruction and support related to care plans and safety.
- Coordinate care with providers, SCPP staff, DME suppliers, assisted living facilities, SNFs, paramedics, pharmacies, administration, and home care coordinators; collaborate with hospital teams and contractors as needed.
- Document falls (with or without injury), medication errors, incident reports, grievances, and other quality measures; participate in quality improvement activities and ongoing professional development.
- Maintain flexible responsiveness to changing workloads; keep leadership informed of problems or issues; contribute to orientation and in-service programs.
Requirements
- Licensure: Current RN license in Michigan
- Education: RN Diploma or Degree from an accredited nursing program; Bachelor's degree preferred
- Experience: Minimum of 1 year working with frail, elderly, or long-term care populations; 1 year of home care experience preferred; 1 year wound care experience preferred
- Certifications: Current CPR certification by the American Heart Association or willing to obtain
- Other requirements: Reliable transportation plus a current Michigan driver's license and car insurance; 18 years of age or older; fully vaccinated or willing to be fully vaccinated against influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis B
- Work environment: Able to perform weekend shifts and travel to patient homes as part of home health visits
