An ISNP (Institutional Special Needs Plan) Nurse Practitioner provides medical care to patients in long-term care facilities such as nursing homes. They focus on managing chronic conditions, preventing hospitalizations, and coordinating care with interdisciplinary teams. Their role includes conducting assessments, developing treatment plans, and working closely with facility staff to improve patient outcomes. This position requires strong clinical skills, geriatric expertise, and the ability to work independently within a facility setting.
To thrive as an ISNP Nurse Practitioner, you need advanced clinical assessment skills, expertise in geriatric care, and a valid nurse practitioner license, often with board certification in adult-gerontology or family practice. Familiarity with electronic health record (EHR) systems and telehealth platforms is important, as these are commonly used for documentation and remote consultations. Strong interpersonal communication, problem-solving abilities, and teamwork are vital soft skills for collaborating with interdisciplinary teams in long-term care settings. These competencies ensure high-quality, continuous care for complex, medically fragile populations residing in skilled nursing facilities.
A typical workday for an ISNP Nurse Practitioner often involves conducting comprehensive health assessments, developing care plans, and managing acute and chronic conditions for residents within a skilled nursing facility. You’ll collaborate closely with nursing staff, case managers, physicians, and sometimes families to coordinate care and facilitate transitions in care settings. The role may also include regular team meetings, documentation in EHR systems, and leveraging telehealth tools for specialist consultations or follow-ups. This work environment emphasizes teamwork, proactive care, and direct patient interaction to improve health outcomes and reduce unnecessary hospitalizations.
