Client Services Navigator
This is a bargaining unit position represented by the OPEIU. All terms and conditions of employment are subject to change in accordance with a collective bargaining agreement.
WORK SCHEDULE: This full-time, regular, non-exempt position is based onsite and may require work during days, evenings, nights, weekends, and at alternate work locations. Standard business hours are generally 10:00 a.m. to 6:00 p.m. This position is on-site, client-facing, and non-remote.
ABOUT ISIS AT STRALEY HOUSE: ISIS at Straley House is a transitional living program serving young adults experiencing homelessness and housing instability. Operated as part of YouthCare, the program provides a safe, inclusive, and affirming residential environment where residents are supported through stable housing, individualized service planning, life skills development, and community connections that promote long-term stability and independence. ISIS is designed to support LGBTQIA+ young adults while welcoming eligible residents who are affirming allies and comfortable living in a queer-focused environment. The program's approach is grounded in trauma-informed care, cultural responsiveness, community-building, and individualized support.
COLLABORATION WITH CENTRALIZED CLIENT SERVICES TEAM: This position works in close collaboration with the Centralized Client Services (CCS) team to support coordinated service delivery, housing navigation, referrals, and access to community-based resources for residents. While the role is housed within ISIS at Straley House and supervised by program leadership, it partners closely with CCS staff to strengthen service planning, housing support, cultural needs planning, and overall coordination of care.
POSITION SUMMARY: Under the supervision of the Program Manager - ISIS, the Client Services Navigator provides direct, trauma-informed services to residents at ISIS at Straley House, with a focused caseload of up to 20 residents. This position supports housing stabilization, individualized service planning, life skills development, cultural needs planning, and transition planning for young adults working toward greater stability and independence.
In addition to direct service responsibilities, the Client Services Navigator supports the Program Manager in strengthening program structure through the coordination of life skills programming, structured resident activities, service schedules, planning tools, and consistent service delivery processes. Primary responsibilities include assessment, individualized service planning, housing navigation, Coordinated Entry for All (CEA) referrals and nominations, connection to community resources, and support with the creation and facilitation of life skills lessons, workshops, and resident programming.
This position works in a multidisciplinary environment and collaborates closely with program leadership, Engagement Specialists, and the Centralized Client Services (CCS) team to support coordinated care, resident engagement, affirming practice, and timely follow-through on service goals. While primarily assigned to ISIS at Straley House, this role may also support life skills programming, structured activities, planning tools, and service coordination practices across YouthCare Housing programs when assigned and in alignment with program needs.
The Client Services Navigator helps residents work toward stable housing, education, employment, health, and independent living while also supporting overall program consistency through structured schedules, planning tools, and coordinated support. The role requires strong organizational skills, sound judgment, flexibility, and the ability to build effective working relationships in support of young adults in crisis as they work toward stability and permanent housing.
ESSENTIAL RESPONSIBILITIES:
  • Manage a focused caseload of up to 20 residents at a time.
  • Conduct comprehensive assessments, develop individualized service plans, and support residents in identifying and working toward housing, education, employment, wellness, and cultural support goals.
  • Maintain and track 30-day, 60-day, and 90-day planning for assigned residents and support consistent implementation and follow-through across the program.
  • Assist residents in navigating housing systems, including Coordinated Entry for All (CEA), housing referrals, landlord communication, document gathering, and transition planning.
  • Collaborate with internal and external partners to support long-term housing placements and other services necessary to support resident stability.
  • Connect residents to healthcare, mental health services, substance use treatment, benefits, education, employment, and other appropriate community resources.
  • Support residents in developing independent living skills, including budgeting, meal planning, organization, household responsibilities, self-advocacy, transportation use, time management, communication, and decision-making.
  • Collaborate with and support the Program Manager in developing, coordinating, and maintaining life skills groups, workshops, structured resident activities, and service schedules.
  • Assist with creating lesson plans, curriculum materials, group outlines, and engagement tools that support resident growth in independent living, housing readiness, and daily functioning.
  • Support the coordination of a consistent weekly and monthly schedule of life skills services, resident groups, and structured activities based on program needs and resident goals.
  • Coordinate with program leadership and residential staff to ensure planned groups, workshops, and activities are prepared, communicated clearly, and carried out consistently.
  • Support the development and maintenance of structured programming calendars across Housing programs, including recurring life skills topics, community-building opportunities, cultural activities, and other meaningful engagement opportunities, as assigned.
  • Help strengthen overall program structure by supporting clear routines, resident expectations, daily and weekly programming flow, and consistency in service delivery within ISIS and, when assigned, across Housing programs.
  • Support and help lead house meetings, resident check-ins, and other structured group spaces in collaboration with program staff.
  • Gather information related to residents' cultural needs and preferences and assist in developing, maintaining, and reviewing cultural needs planning in alignment with program expectations and individualized service planning requirements.
  • Identify, coordinate, and maintain culturally responsive, affirming, and community-based activities relevant to resident interests and needs, including opportunities that support LGBTQIA+ residents while maintaining inclusive programming for all eligible residents.
  • Develop and maintain service planning tools, templates, trackers, and workflows that support documentation completion, resident progress monitoring, cultural needs planning, and coordination between case management and residential staff.
  • Work closely with Engagement Specialists, the CCS team, and program leadership to align daily support, resident goals, service schedules, and follow-up actions.
  • Assist with intake, transition, and exit coordination, including preparing for move-ins, orienting residents to the program, and supporting successful transitions toward stable housing.
  • Maintain clear, timely, and professional participant contact notes, intake paperwork, service plans, cultural planning documentation, progress updates, statistical records, and other required documentation in accordance with confidentiality laws, agency policy, contracts, and quality assurance expectations.
  • Monitor documentation timelines, resident planning milestones, service participation, and follow-through, and communicate gaps or concerns to program leadership as appropriate.
  • Complete Performance and Quality Improvement (PQI) tasks, support accreditation standards, and adhere to all agency policies and procedures.
  • Comply with the agency's mandated reporting requirements and all other applicable reporting responsibilities.
  • Assist with orientation, onboarding support, and training reinforcement for new staff or on-call staff related to service coordination, documentation practices, life skills programming, and resident planning tools, as needed.
  • Participate in regular case reviews and support discharge and transition planning for residents moving to the next stage of housing stability.
  • Manage and maintain confidential client files as required by contracts, agency policy, and quality assurance expectations.
  • Attend and participate reliably in training and professional development as needed for the role and the needs of the agency, including program requirements, DEI&J, supervision, and other identified areas.
  • Contribute to YouthCare's DEI&J culture by creating at least one SMART Goal related to DEI&J within your control to deliver.
  • Provide transportation and/or support for appointments as needed.
  • Provide support with life skills programming, structured activities, service planning tools, documentation workflows, or resident engagement needs across YouthCare Housing programs when assigned by program leadership.

QUALIFICATIONS -
MINIMUM REQUIREMENTS:

  • Bachelor's degree or master's Degree in counseling, social work, human services, or a related field.
  • At least one (1) year of experience working with at-risk youth or young adults in a crisis, during outreach efforts, or in a residential/community-based setting OR four (4) years of full-time work experience directly working with, managing, or overseeing vulnerable populations in a related field.
  • Experience and credentials must comply with program contractual requirements, including but not limited to the Washington State Department of Children, Youth, and Families (DCYF) background clearance process and must be at least 21 years of age. DCYF Clearance must be maintained for the entirety of employment when working with under-18 clients.

PREFERRED REQUIREMENTS:
  • Prior case management experience.
  • Two (2) or more years of experience successfully working with vulnerable young people or young adults in crisis, during outreach efforts, or in a residential setting.
  • Experience developing, supporting, or facilitating life skills groups, workshops, curriculum, or psychoeducational activities for young people.
  • Experience supporting housing navigation, documentation systems, program coordination, or structured programming in a residential or shelter setting.
  • Training or coursework in human development, mental health, chemical dependency, housing navigation, residential care, education, group facilitation, or culturally responsive service delivery.

CONDITIONS OF EMPLOYMENT:
  • Must be able to provide I-9 supporting documentation for employment.
  • Ability to provide a negative TB Test (results must be within one year of hire date).
  • Employees who drive YouthCare vehicles as a requirement for their position must be at least 21 years old, possess a valid Washington State Driver's License, meet minimum safe driving criteria established by the agency and our insurance company which includes completion of Van Certification trainings. Drivers under the age of 25 must have been licensed for at least 3 years and those operating vehicles with a capacity of 15 or more passengers must be at least 25 years old.
  • Ability to obtain a current Food Handler's Card
  • Successful completion of pre-service training courses as required by contracting agencies. Training such as CPR & First Aid (Adult/Child), Bloodborne Pathogens, and Behavior Management.

  • Ability to maintain professional boundaries, respond therapeutically in crisis situations, and de-escalate conflict.

PHYSICAL CONSIDERATIONS:
  • Ability to perform a range of physical motions, exerting up to 50 pounds. Lifting and carrying up to 50 pounds; standing, walking, sitting for long periods of time, kneeling, squatting, and stooping; running for brief periods of time; and go up and down stairs.
