Iron Gate Health & Pain Center is seeking a compassionate and dedicated Nurse Practitioner to join our dynamic medical team in Kingston, New York. Work Environment

This role is primarily onsite, allowing for direct and meaningful interactions with patients in a variety of settings, including their homes. The workweek consists of five days, Monday through Friday, totaling 40 hours per week, providing work-life balance while delivering high-impact care.

Key Responsibilities

• Conduct thorough physical examinations to assess patient health and obtain detailed medical histories.
• Diagnose and effectively manage both acute and chronic illnesses, applying evidence-based practices.
• Develop, implement, and regularly reassess individualized treatment plans tailored to the unique needs of each patient.
• Order, interpret, and follow up on diagnostic tests, including labs and imaging, to ensure comprehensive evaluation.
• Provide ongoing patient education on disease prevention, self-management strategies, and health maintenance.
• Collaborate closely with physicians and other healthcare providers to ensure coordinated care and optimal patient outcomes.
• Maintain meticulous and accurate medical records in accordance with facility protocols, ensuring compliance with healthcare regulations.
• Participate in team meetings and case reviews to discuss treatment progress and optimize care strategies.

Patient Population / Client Focus

The Nurse Practitioner will primarily work with patients suffering from chronic pain conditions, including but not limited to arthritis, back pain, fibromyalgia, and neuropathic pain. The ideal candidate will demonstrate empathetic communication and a comprehensive understanding of pain management techniques, focused on improving quality of life and functional ability for our diverse patient population.

Qualifications

• Bachelor’s Degree in Nursing and a Master’s Degree as a Nurse Practitioner.
• Current Registered Nurse Licensure and certification as a Nurse Practitioner.
• A minimum of 2 years of experience in pain management or performing physical examinations.
• Proficiency in clinical skills, with a strong ability to diagnose, develop treatment plans, and implement patient care strategies.
• Strong interpersonal and organizational skills, with a commitment to teamwork and collaboration.
• Ability to work independently and manage time effectively in a fast-paced environment.
• Current certifications in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) are required.

Preferred Experience

Experience within a concierge medicine context or prior work in a pain management clinic is highly advantageous. Familiarity with electronic health records (EHR) systems and a robust understanding of pain assessment tools will further enrich your ability to deliver care.

Compensation and Benefits

The salary range for this position is between USD 150,000 and 160,000 annually, depending on experience. In addition to a competitive salary, we offer a comprehensive benefits package that includes:
