Job Description

Supervisor of social worker, students and interns. Provides appropriate and timely discharge from the nursing facility. Arrangements for follow-up care and referrals to community resource as needed. This responsibility is to be carried out with an understanding of Medical Social Work concepts, practices and procedures.


Qualifications

Education:

  • Masters Degree in Social Work

Experience:

  • Extensive fast paced discharge- planning experience.
  • Supervisory experience preferred

Certification and Licensure:

  • LCSW Preferred

Skills:

  • Demonstrate excellent written and spoken skills
  • Bilingual Spanish/English, preferred
  • Able to work varying schedules, including weekends and evenings
  • Computer knowledge sufficient to access data, run reports, update, clinical programs. Working knowledge of Microsoft Windows, Excel and PowerPoint.
  • Experience and judgment to plan and implement
