Job description

Overview
The RN Transfer Center, under the supervision of the Director and/or Clinical Manager, is responsible for facilitating admissions/transfers to and within NAH. S/he maintains the Physician Referral Line service to provide timely access to area specialists as needed. S/he will direct, delegate, and coordinate the process needed for patient access NAH. S/he follows accepted nursing practice, policy, and procedures. S/he performs procedures that are within the scope of department and nursing practice. S/he seeks assistance and support when necessary, assists in data collection, and facilitates admissions or transfer of patients for patient care. Maintains accountability for own practice and those functions s/he delegates. Within a specified work period, plans and coordinates patient placement and physician consults.
Responsibilities
Administrative
* Updates Daily Bed Poll in accordance with Emergency Management Systems.
* Clerical duties to include: use of computer systems and applications, faxing, scanning, printing, etc.
* Assists with ensuring the physician on call roster is kept up-to-date.
Patient Care
* Obtains pertinent medical history and information from referring physician.
* Prioritizes and problem solves to meet changes in patient needs with regard to admission/transfer/resource availability.
* Makes appropriate specialist referrals per the Single Provider Acceptance policy.
* Collaborates with House Supervisor to ensure the level of care and resources are available for patient acceptance.
* Communicates with referring facilities regarding updates, changes, transport, etc.
* Coordinates wait and return visits.
Data/Reporting
* Ensures that data entry into Cerner applications, and Transfer Center software is accurate and timely.
* Completes a thorough and timely record of all admissions and transfers
* Reports hospital deaths to the PBX operator and House Supervisor, per FMC policy.
* Assists in data collection and reporting as needed.
* Responsible for sending out inpatient STEMI alerts in a timely manner.
Bed Placement
* Collaborates with the House Supervisor in the placement or transfer of patients.
* Makes appropriate admission status assignments, taking into account diagnostic tests, labs, etc.
Compliance/Safety
* If required for the position, ensures all certifications and/or licenses are up-to-date and valid prior to expiration dates.
* Completes all company mandatory modules and required job-specific training in the specified time frame.
* Responsible for reporting any safety-related incident in a timely fashion through the Vigilanz tool; attends all safety-related training programs; performs work in a safe manner; monitors work environment for possible safety issues and ensures others are also performing work in a safe manner.
* Adheres to regulatory and organizational standards, to include policies and procedures.
Qualifications
Education:
Graduate of accredited School of Nursing- Required
BSN- Preferred
Certification & Licensures:
Active, unrestricted, Arizona RN License or valid, participating compact license- Required
BLS (American Heart Association)- Required
Experience:
Three years acute care experience- Required
Call center/Transfer Center experience- Preferred
