Evergreen Post Acute in beautiful Smyrna, DE is looking for a Full Time Registered Nurse Assessment Coordinator (MDS/RNAC) to join our Team!

Job Responsibilities:

The RNAC is responsible for the processing related to the assessment of patients and residents in a health care facility.

The RNAC will assess patients, asking patients to report how they feel, providing regular reports to doctors and other medical professionals, billing, and working with the MDS (medical decision support) system to ensure that all conditions are treated in the best possible way.

The Registered Nurse Assessment Coordinator will assist with patient’s examinations and/or questions.

The RNAC will implement and interpret organizational policies and procedures, and nursing standards and regulations.

The RNAC will assist with preparing the departmental fiscal budget.
