Key Responsibilities

  • Initiate discharge planning upon patient admission, conducting discharge sessions to ensure clients have a clear understanding of their coping strategies and transition resources.
  • Manage crises effectively during sessions and support milieu staff in patient interactions, maintaining a high level of clinical quality through teamwork.
  • Complete all required clinical documentation in a timely manner, ensuring that it is done accurately at the time of service.
