This role is primarily remote, enabling you to work from the comfort of your own home while contributing to impactful outcomes for our clients in the hospice center setting. While the bulk of your work will be conducted during regular business hours, flexibility is required, as you may occasionally need to participate in evening or weekend events related to community outreach or special programs. Our team collaborates closely using shared digital platforms to ensure effective communication and support for one another.

• Conduct thorough, comprehensive assessments to identify clients' social, emotional, and psychological needs, ensuring tailored support.
• Provide individual and group counseling sessions utilizing evidence-based therapeutic approaches to facilitate healing and growth.
• Develop and implement personalized service plans that align with clients’ unique goals and objectives, monitoring progress and adjusting strategies as needed.
• Advocate for clients' rights and navigate them through access to necessary resources, focusing on social justice issues that affect the community.
• Contribute to program development and strategic planning, including evaluation and grant reporting to enhance our service offerings.
• Collaborate with local organizations, government agencies, and volunteers, leveraging strengths and resources to maximize service impact.
• Engage actively in team meetings, training sessions, and community outreach activities to foster a culture of learning and engagement.

In this role, you will work closely with a diverse clientele that includes individuals facing a variety of challenges, from mental health issues to social injustices. • A Master’s degree in Social Work from an accredited program is required.
• An active state license as a Licensed Clinical Social Worker (LCSW) is essential.

• Prior experience in a nonprofit organization or a similar agency is preferred, particularly in community-focused roles.
