• Conduct comprehensive patient intakes, focusing on diagnostic and functional evaluations to inform treatment planning.
• Provide brief, focused therapeutic interventions for individuals seeking mental health support.
• Collaborate closely with primary care providers to identify patients requiring specialized mental health services, equipping them with suitable referrals.
• Initiate and follow up on treatment interventions, adjusting plans as necessary to enhance patient outcomes.
• Develop relapse prevention plans and assist patients in maintaining stable functioning in their daily lives.
• Monitor patients for behavioral health changes and work proactively to prevent deterioration.
• Document patient progress rigorously, keeping records that support clinical assessments and treatment plans.
• Educate and empower patients in self-management and coping strategies related to their mental health treatment.
• Collaborate with clinic management and team members to provide input on program evaluation and improvement initiatives.

The annual salary for this position ranges from USD 87,000 to 105,000, commensurate with qualifications and experience.
