As part of a multidisciplinary team, you will collaborate with other healthcare professionals via telehealth platforms, maintaining effective communication to deliver integrated care to our patients.

• Reporting directly to the Social Work Manager, you will evaluate and address psychosocial issues impacting your patients, employing individual clinical social work skills to support families in overcoming barriers related to mental and physical health challenges.
• Adhere to the National Association of Social Workers (NASW) Code of Ethics, ensuring that your practices align with professional standards and our organization’s corporate values of respect, honesty, and fairness.
• Conduct thorough bio-psychosocial assessments to identify barriers to treatment and enhance the overall health status of patients, ensuring appropriate resource utilization.
• Collaborate with medical and support staff to create comprehensive care plans, speeding up treatment compliance and improving patient outcomes.
• Design and implement treatment strategies, including individual and group therapies, while continuously analyzing the progress of treatment to ensure optimal health improvement and empower patients' healthcare decisions.
• Document all social work activities within patient medical records in accordance with departmental standards, ensuring accurate record-keeping for continuity of care.
• Provide educational resources and emotional support to patients and their families, guiding them in understanding and managing their treatment plans effectively.
• Assess the advocacy needs of patients and their families, coordinating community resources that promote self-determination and enhance social support networks.
• Maintain up-to-date knowledge of available community resources, facilitating easy access when necessary to support our patients and their families.
• Engage in ongoing professional development activities to enhance your clinical expertise and stay abreast of developments in the social work field.
• Comply with all relevant laws, regulations, and organizational policies to help maintain a safe and compliant work environment.

Your work will predominantly focus on individuals experiencing significant psychosocial stress due to terminal or chronic illnesses within the hospice care setting. You will be helping patients and their families navigate these challenging times by providing empathetic counseling and resource facilitation aimed at improving their overall quality of life.

• A Master’s degree in Social Work from an accredited institution, along with a current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of one year of clinical experience, ideally within health or mental health settings, particularly catering to populations that reflect the needs of our patients.
• Solid understanding of human development, sociocultural factors, and aging processes that influence patient care, particularly in end-of-life scenarios.
• Ability to work collaboratively within a healthcare team, demonstrating strong interpersonal and communication skills essential for effective patient interactions.

• Prior experience working within hospice or palliative care environments.
• Enhanced skills in evidence-based therapeutic modalities suitable for addressing diverse patient needs.
• Knowledge of community resources relevant to patient care and advocacy efforts.

The annual salary for this position ranges from USD 85,000 to 100,000, commensurate with experience.
