• Conduct comprehensive assessments to evaluate the mental health status of patients, employing the Diagnostic and Statistical Manual of Mental Disorders, as necessary, to inform individualized care plans.
• Utilize evidence-based practices to develop and implement treatment plans aligned with identified patient goals, with a strong focus on cultural sensitivity and the diverse needs of each patient.
• Monitor and evaluate each patient’s progress regularly, providing timely interventions and adjustments to care strategies as needed.
• Prepare and maintain accurate treatment records, clinical documentation, and reports using contemporary technological tools, ensuring compliance with regulatory standards.
• Facilitate collaboration with patients and their families to create comprehensive treatment plans, ensuring alignment with the desired outcomes and agreed-upon interventions.
• Coordinate with other healthcare providers to establish strong referral networks and facilitate access to additional support services as required.
• Engage in mentorship activities by providing education and guidance to students and interns, fostering a culture of learning within the organization.
• Participate in peer reviews, clinical collaboration, and team audits to contribute to continuous quality improvement and team development.

Your primary focus will be on individuals and families facing the challenges associated with cancer diagnoses. This specialized population necessitates a compassionate approach aimed at addressing emotional and psychological obstacles, with strategies designed to improve mental health outcomes and facilitate coping mechanisms.

• A Master’s degree in Social Work from an accredited program.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the State of New York.

While not mandatory, candidates with experience in an outpatient or school-based mental health setting focusing on oncology patient care are preferred. Proficiency in evidence-based therapeutic modalities and familiarity with telehealth platforms will enhance your success in this role.
