Gotham Enterprises Ltd is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our compassionate team at our Hospice Center in Bloomington, Illinois, on a full-time remote basis. In this critical role, you will provide essential support to seniors facing the challenges of advanced illnesses, ensuring their emotional, psychological, and social well-being is prioritized while they receive hospice care.

This full-time role is predominantly remote, with occasional visits to our Hospice Center and clients in their homes as necessary. The standard work hours are Monday to Friday, from 8:00 AM to 4:30 PM, although flexibility will be required to accommodate the needs of our clients. You may need to work evening or weekend hours occasionally and be available for on-call situations to address urgent client needs.

• Conduct holistic psychosocial assessments of seniors, evaluating their emotional, mental, and social needs to tailor care plans.
• Collaborate with clients and their families to create personalized care plans that align with their values and preferences, integrating both medical and psychosocial elements.
• Provide counseling and emotional support, assisting families in coping with difficult transitions and challenges during the hospice process.
• Advocate for clients' rights and help them access available community resources and services to enhance their quality of care.
• Work within an interdisciplinary team, including nurses, doctors, and hospice aides, to coordinate care and develop strategies for improved client outcomes.
• Conduct regular follow-up visits to monitor the well-being of clients, ensuring their safety and comfort in the home environment.
• Foster effective communication among family members, helping to mediate discussions regarding the care and concerns of their loved ones.
• Educate clients and their families about available resources, support groups, and activities that promote social engagement and emotional well-being.
• Maintain thorough documentation of assessments, care plans, and all professional interactions with clients in compliance with ethical standards and regulations.

Your primary focus will be on seniors in hospice care, who are navigating the complexities of advanced illnesses. This population often requires sensitive, timely interventions to address unique challenges including end-of-life planning, grief, and loss. You will play an essential role in ensuring clients and their families receive compassionate care and support tailored to their specific needs.

• A current and valid state-issued license as a Clinical Social Worker (LCSW) is required.
• A Bachelor’s Degree in Social Work or a closely related field is mandatory; a Master’s Degree is highly preferred.
• Minimum of two years of relevant experience in social work, particularly within geriatric or hospice settings, is necessary.
• Strong understanding of social work principles, aging-related issues, and community resources available to clients.
• Exceptional interpersonal and communication skills, combined with a genuine empathy for seniors and their families.
• Proficiency in electronic health record (EHR) systems to document assessments and care plans efficiently.

• Experience working directly with seniors and their families in a hospice or palliative care setting.
• Knowledge of and experience in navigating healthcare policies and community services available to seniors.
• Previous involvement in interdisciplinary team-based care approaches.

This position offers a competitive salary ranging from USD 60,000 – 100,000 per year, commensurate with experience and qualifications.
