Carry out thorough assessments and diagnoses of patients as needed, employing the Diagnostic and Statistical Manual of Mental Disorders and other clinical tools to evaluate mental health conditions, while creating individualized care plans that consider cultural diversity and specific patient needs.
Continuously monitor and assess patient progress, employing evidence-based interventions tailored to each client’s mental health diagnosis, and adapting strategies to effectively support their needs.
Maintain accurate treatment records and clinical documentation in accordance with regulatory requirements and department protocols, leveraging current healthcare technologies to ensure timely and efficient record-keeping.
Collaborate with patients and their families in developing comprehensive treatment plans, aimed at achieving desired health outcomes, involving both the client and family in goal-setting when suitable.
Coordinate referral options and facilitate communication between clients and other treatment providers, enhancing the continuity of care and establishing productive relationships with referral agencies.

QualificationsA Master’s degree in Social Work from an accredited institution.
