Work Environment

This position is primarily remote, offering the flexibility to work from the comfort of your home while still receiving support from our collaborative team. • Proactively anticipate areas of patient safety concerns and mitigate risks by implementing effective interventions and symptom reduction strategies.

  • Facilitate referrals to additional services and coordinate with other healthcare professionals to ensure a seamless continuum of care for the patient.

  • Engage in educational opportunities by mentoring students, interns, and other learners who are keen to develop their skills in social work and mental health care.

  • Participate in clinical collaboration activities such as peer reviews, chart audits, and mentoring designed to elevate overall team performance and professional development.

Patient Population / Client Focus

Our patient population includes individuals from varied backgrounds, facing a range of mental health issues that often intersect with significant medical conditions. At Tiny Steps, we approach each case with a deep understanding and sensitivity, working to create inclusive and responsive care strategies that not only address mental health but also consider the comprehensive needs associated with end-of-life care.

Qualifications
  • A Masterโ€™s Degree in Social Work from an accredited institution.

  • A current license as a Licensed Clinical Social Worker (LCSW) from the State Board of Social Work Licensure.

  • Demonstrated ability to provide culturally competent care and a solid understanding of human development and aging processes.

Preferred Experience
  • Experience in an outpatient mental health setting, ideally within hospice or palliative care.

  • Familiarity with evidence-based treatment modalities and individualized care planning methodologies.

  • Prior experience mentoring or supervising junior staff or interns.

Compensation and Benefits
