You will be working in a climate-controlled setting, primarily from the convenience of your home, with occasional visits to our hospice facility in Joliet, Illinois. This role emphasizes collaboration with fellow healthcare professionals and department members, fostering a supportive environment where care delivery is improved through teamwork and shared expertise.

• Conduct comprehensive assessments of patients to diagnose their mental health status, utilizing tools such as the Diagnostic and Statistical Manual of Mental Disorders. Personalize each patient’s care plan to ensure it reflects their unique cultural backgrounds and individual needs.

• Regularly monitor and evaluate patient progress, employing evidence-based strategies to address mental health issues effectively.

• Maintain accurate and timely documentation of treatment records and clinical information, complying with regulatory guidelines and departmental mandates.

• Collaborate with patients and their families to formulate a comprehensive treatment plan aimed at achieving desired health outcomes while respecting their preferences and goals.

• Lead therapy sessions by developing targeted treatment objectives, applying effective interventions that prioritize patient safety and symptom management.

• Serve as a vital link between patients and additional healthcare providers, facilitating referrals and ensuring continuity of care through effective communication with various agencies.

• Foster the growth and learning of future social workers by providing education and mentorship to students, interns, and volunteers participating in our programs.

• Engage in peer-to-peer review and collaboration under the guidance of a clinical supervisor to promote best practices within the team.

This role primarily serves individuals and families impacted by a cancer diagnosis, offering therapeutic support that addresses the emotional and psychological challenges associated with illness. You will provide empathetic guidance and develop treatment plans tailored to the complexities of your patients' unique circumstances, ensuring they receive the holistic care needed during their critical journeys.

• A Master’s degree in Social Work is required.

• A current and valid Licensed Clinical Social Worker (LCSW) license in the State of Illinois is mandatory.

• At least two years of experience in a clinical social work setting, preferably focused on outpatient therapy, hospice care, or oncology.

• Familiarity with evidence-based therapeutic techniques and mental health assessment tools is advantageous.

The annual salary range for this position is between $90,000 and $105,000, commensurate with experience.
