As a Licensed Clinical Social Worker at Conejos Community Health Care Center, you will:

• Conduct comprehensive psychosocial assessments to identify the emotional, social, and educational needs of students.
• Provide individual counseling and facilitate group therapy sessions focused on issues such as anxiety, grief, and adjustment concerns.
• Work closely with educational staff to develop tailored interventions that support students' mental health and academic success.
• Collaborate with local community organizations to ensure students and their families have access to necessary resources, especially following critical transitions or discharges.
• Offer education and support to parents and caregivers regarding community resources and treatment options available to their child.
• Serve on school committees, contributing your expertise in social work to enhance policy and program development.
• Document all interventions and client interactions in accordance with state and federal guidelines, ensuring compliance with hospital policies.

Your primary focus will be on students in a school setting, addressing various psychosocial challenges that may affect their ability to succeed academically and socially. This role requires a keen understanding of the issues faced by children and adolescents, including but not limited to learning disabilities, family dynamics, trauma, and socio-economic challenges.

To qualify for this position, candidates must possess:

• A Master’s Degree in Social Work from an accredited institution.
• An active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong knowledge of mental health disorders, crisis intervention techniques, and educational systems.
• Exemplary communication and interpersonal skills, with a capacity for building relationships across diverse populations.

While a minimum of two years of experience in a healthcare or school-based setting is preferred, candidates who demonstrate a strong desire to support students and have relevant internship or volunteer experience will also be considered. Familiarity with medical terminology, mental health conditions, and community resource referrals will enhance your effectiveness in this role.
