Job description

Role Overview: The Behavioral Health Care Manager (BHCM) is responsible for providing specialized behavioral health care coordination and care management services to support members in achieving optimal mental health outcomes. This role serves as a key point of contact for members, caregivers, providers, and external partners, ensuring continuity of care across all service levels. The BHCM reports to the Behavioral Health Manager in Integrated Health Care Management and supports members through engagement, assessment, care planning, and service coordination.

Work Arrangement:

  • This role may be remote, and the associate must be in Louisiana (LA)
  • Travel may be required within a 50-mile radius to engage with members, providers, and community partners.
  • Associates must have access to a reliable high-speed internet connection to support daily responsibilities, with a minimum download speed of 50 Mbps and an upload speed of 5 Mbps. Fully remote associates residing in states where internet service is required by contract, law, or regulation may be eligible for reimbursement.

Responsibilities:

  • Coordinate behavioral health care services for members, including assessment, care planning, and goal setting
  • Engage members, caregivers, and providers to support individualized treatment and care plans
  • Serve as a single point of contact and advocate for members in the care coordination program.
  • Facilitate care transitions across levels of care and ensure continuity of services
  • Collaborate with state agencies, community-based organizations, providers, and practitioners to coordinate care.
  • Act as a liaison between members and external entities, including judicial systems and Department of Health and Hospital (DHH) representatives
  • Support judicial-related activities by serving as a point of contact for judges and court personnel and attending court proceedings when required
  • Maintain familiarity with drug court, juvenile court, family court, and criminal court processes, and provide education on access to services
  • Serve as liaison for Local Government Equity programs and Permanent Supportive Housing (PSH) initiatives, addressing inquiries, barriers, and resolution needs
  • Participating in Human Services Interagency Council (HSIC) meetings as requested
  • Ensure delivery of high-quality, cost-effective care while managing resources appropriately.

Education & Experience:

  • Master's degree from an accredited institution in a behavioral health-related field (e.g., Social Work, Counseling, Marriage and Family Therapy)
  • 7 years of experience in behavioral healthcare operations, including case management, mental health, or related fields
  • Experience working within Medicaid and Medicare behavioral health environments
  • Background in utilization management, care coordination, and population health management
  • Demonstrated experience working with interdisciplinary healthcare teams
  • Experience using electronic health records (EHR) and data analytics tools
  • Experience developing, implementing, or supporting behavioral health programs and initiatives

Licensure:

  • Active and unrestricted licensure required in one of the following: Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT)
  • Eligibility to obtain Certified Case Manager (CCM) certification within one year of hire
  • Valid driver's license and current auto insurance required
