The Senior Licensed Clinical Social Worker (LCSW) plays a pivotal role in enhancing patient care and outcomes within MemorialCare's Hospice Center. This position involves remote work with a dedicated focus on providing psychosocial support to patients and families during a critical stage of their healthcare journey. The LCSW is tasked with removing barriers that hinder safe transitions through the continuum of care, ensuring that patients are empowered to take an active role in their treatment options and decisions.
