New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, 24-hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee and St. Lucie counties. In addition, New Horizons assists 22,000 students in area schools, and we work closely with the courts, law enforcement, jails, and hospitals to help improve the health of individuals and the quality of life in our community.


 New Horizons is seeking for a Clinical Intake Lead Therapist  who will be responsible for delivering high-quality intake,
assessment, and care-coordination services within a fast-paced behavioral health setting. This role conducts comprehensive biopsychosocial assessments, develops individualized treatment plans, completes risk assessments, and administers mental health and substance use outcome measures. The therapist ensures clients are connected to appropriate levels of care and linked to community resources. 

In addition, the Clinical Intake Lead Therapist holds supervisory responsibilities, overseeing clinical intake counselors and coordinating master's-level interns in social work and mental health programs. May include rotating coverage for crisis follow-up or coordination with Crisis Stabilization

Units. 


As a Clinical Intake Lead Therapist, your responsibilities will include:

  •  Conduct comprehensive biopsychosocial assessments for clients of all ages. 
  •  Complete suicide, self-harm, violence, and substance use risk assessments. Complete Safety Plans with clients and collaterals. 
  •  Develop individualized treatment plans based on client needs, strengths, and goals.
  •  Maintain accurate, timely, and compliant documentation in accordance with agency, state, and federal standards. 
  •  Provide referrals to internal programs, external providers, and community resources 
  • Collaborate with multidisciplinary teams including case managers, prescribers, crisis staff, and community partners.
  • Coordinate follow-up services for clients discharged from Crisis Stabilization Units or referred after medication management appointments. 
  •  Provide intake services through scheduled appointments, walk-ins, and same-day access. 
  •  Engage clients and families in treatment planning and resource navigation. 
  •  Conduct follow-up check-ins to ensure continuity of care. 
  •  Provide leadership, training, guidance, and oversight to clinical intake counselors 
  • Review and approve all documentation from intake counselors and other clinical departments as needed 
  • Coordinate and supervise master's program interns from social work and mental health programs, including onboarding, training, and evaluation. 
  • Ensure adherence to ethical standards, agency policies, and professional best practices within the intake team 
  • Maintain current knowledge of evidence-based practices, community resources, and regulatory requirements. 
  •  Participate in team meetings, case consultations, and clinical supervision. 

Requirements

REQUIRED:

  •  May include rotating coverage for crisis follow-up or coordination with Crisis Stabilization Units. 
  • Active licensure with the Florida Board of Clinical Social Work, Marriage & Family Therapy, and Mental Health Counseling (e.g., LCSW, LMHC, LMFT). 
  •  Experience conducting clinical assessments and working with diverse populations 
  • Strong knowledge of mental health and substance use disorders, crisis intervention, and care coordination. 
  • Ability to supervise staff and interns or willingness to obtain supervisory qualifications as required by the Florida Board

PREFERRED:

  •  Experience in community mental health, crisis services, or intake environments.
  • Familiarity with electronic health records and outcome measurement tools.
  • Experience in a supervisory role.
