Job description

RN Clinical Rehab Spec (FLEXI)

The Registered Nurse Clinical Rehabilitation Specialist is responsible for all the nursing needs in the Cardiopulmonary Rehabilitation department of the Lifestyle Center. Specifically, the Clinical Rehabilitation Specialist is responsible for identifying the patient's alterations in health, assessing the patient's human response to actual and potential health problems, and implementing nursing interventions based on theoretical concepts from nursing and other arts and sciences to achieve outcomes for the patient that facilitate resolution of alterations in health, and actual and potential health problems.

Essential Duties and Responsibilities:

  • Coordinate clinical care, lead exercises classes, and supervise the education of cardiopulmonary rehab patients.
  • Coordinate referrals to the rehab program including communication with physicians and medical staff.
  • Schedules patients for program admittance based on demands of department and/or patient needs.
  • Obtains necessary medical records for review and conducts patient interview/assessments to obtain necessary clinical data for program admittance.
  • Proficient in clinical assessment, vital sign monitoring, EKG monitoring and rhythm interpretation and medication reconciliation of cardiopulmonary participants.
  • Develop exercise prescriptions, individualized goals and Individual Teaching Plan (ITP) for cardiopulmonary patients according to program guidelines and in compliance with ACSM, AACVPR and CMS requirements.
  • Ability to interpret patient questionnaires pertaining to exercise, quality of life, performance of ADLs, and mental health to identify and implement plan of care.
  • Proficient in use of hospital EMR and Cardiopulmonary telemetry system.
  • Provide input into the development of policies and procedures for the program and assist the coordinator with various administrative duties and program responsibilities.
  • Be knowledgeable in program requirements according to CMS and AACVPR.
  • Communicate with site physician, cardiac and pulmonary rehab medical directors, and patient's physician as needed for continuum of care and intervention(s) as necessary.
  • Implement and teach educational lectures for cardiac and pulmonary patients.
  • Understanding of appropriate ICD codes and/or diagnosis, insurance verification, and billing CPT codes for program entry and coverage. Assisting in verification and billing issues/denials/claims if hospital billing department needs clarification and/or is unavailable.
  • Maintain proper inventory of department supplies, crash-cart, and educational materials.
  • Provides patients with necessary resources/support to ensure health improvement and disease prevention and treatment (Registered Dietitian, CDE, social worker/mental health consultant, physical therapy, physicians, etc.)
  • Maintain accurate and up-to-date appropriate documentation patient files for Phase II participants: Referral/order, Goal setting, Exercise Prescription, Education, Outcomes, etc.
  • Reviews class census and acuity to provide appropriate staffing and patient safety.
  • Assist in patient education lectures as needed within the department and community.
  • Act as Code Team leader in the event of an emergency.
  • Conducts and documents equipment checks and performs preventative maintenance on necessary equipment utilized within department.
  • Assist in managerial duties in the absence of the Supervisor.
  • Participate in community events to provide education and awareness of disease risk factors and benefits of exercise as needed.
  • Attend department staff training of required competencies and emergency/mock codes necessary to perform patient care.
  • Assist with the training and supervision of exercise physiologists, student interns and volunteers.
  • Maintain contact with physician offices, internal CRMC departments, and staff concerning patient needs.
  • Attend required hospital-wide orientations, meetings, in-services, and HLC/Occupational Health.
  • Demonstrate a commitment to flexible work schedule when necessary to ensure safe patient care.
  • May perform other duties as assigned for department improvement and patient care.

Supervisory Responsibilities:

Reports to: Cardiopulmonary Rehab Supervisor and/or Director, Community Health Services.

Supervises: Exercise Physiologists, Student Interns and Volunteers in absence of Supervisor.

Responsibilities: Assessing patients, treatments, and goal planning; and assist in overseeing policies and procedures for the department; Acting as code team leader in emergencies.

Education and Experience:

Minimum Required Education: Graduation from an accredited Registered Nurse program in the State of Virginia

Preferred Education: Associates, Diploma, or Bachelor of Science in nursing

Experience: 2+ years in the ICU, CCU, Emergency Department, Cardiac Telemetry Unit, or Cardio Rehab facility, plus 1+ years in telemetry monitoring and/or stress testing

Certificates, Licenses, Registrations:

Applicants must be registered to work as a Registered Nurse in the State of Virginia, and must have current BLS and ACLS certifications.
