Job Title: Physical Therapist (PT)
Location: Aaron Manor Nursing & Rehabilitation Center – Fairport, NY
Schedule: Full-Time
Pay: Up to $50/hr + Shift Differentials

Summary:

The Physical Therapist provides skilled physical therapy services to residents in accordance with professional standards, facility policies, and state and federal regulations. This role is responsible for evaluating, treating, and developing individualized therapy plans that support residents in achieving their highest level of function and independence

Responsibilities:

  • Evaluate residents and develop individualized physical therapy treatment plans.

  • Perform patient screenings, annual evaluations, and nursing referrals to determine therapy needs.

  • Utilize standardized assessments to support the need for skilled physical therapy services.

  • Provide therapeutic interventions and treatment modalities, including but not limited to:

    • Diathermy

    • Electrical Stimulation (E-Stim)

    • Ultrasound

  • Monitor resident progress and adjust treatment plans as appropriate.

  • Communicate resident status, progress, discharge planning, and follow-up needs with the Director of Rehabilitation and interdisciplinary care team.

  • Coordinate care with physicians, nurses, and other healthcare professionals.

  • Educate residents and family members on therapy programs and maintenance plans.

  • Complete all required documentation and billing accurately and timely in accordance with regulatory and facility requirements.

  • Attend facility and department meetings as requested.

Qualifications:

  • Graduate of an accredited Physical Therapy program.

  • Bachelor's Degree in Physical Therapy or higher.

  • Current Physical Therapist license in the state of New York.
