What We Are Looking For

The Population Health RN will actively participate in the review, analysis and delivery of care to patients' organization wide. Utilize reporting to identify and actively engage in patient outreach. Responsible for quality planning and coordination of patient care. Responsible for accurate and timely tracking of productivity statistics, program status, challenges, updates, and developments. Utilizes knowledge of basic nursing principles, practices, and ethics to perform duties within the RN Scope of Practice. Performs other duties as needed.

Schedule: 5 days per week, 8 hours per day, Mon-Friday 8am-4:30pm, 30min lunch | Location: Brier Clinic, San Bernardino, CA

ESSENTIAL FUNCTIONS AND DELIVERABLES

  • Effectively utilizes RN scope of practice as it pertains to coordination of quality care.
  • Patient Education - Plans, implements, and evaluates educational programs for the patients, families and other health professionals pertaining to specific illness.
  • Explains illness and answers questions regarding care from patient and family members.
  • Provides effective patient/family education regarding disease management.
  • Quality Management - Participates in clinical quality improvement designed to improve quality of care. Actively works on the development of standards of care and evaluating the quality of care.
  • Demonstrates the knowledge and skills necessary to perform assessments, plan and implement care, and evaluate educational programs for the patients and families.
  • He/she can apply simple and complex techniques required as well as the expertise to use appropriate equipment in the efficient and effective delivery of nursing care to the patient/family as needed within the RN Scope of Practice.
  • Schedules outpatient procedures and assists with obtaining authorizations.
  • Provide clinical oversight to Population Health staff. Review clinical assessment and guides clinical decision making
  • Plan, organize, and participate in population health pop-up vaccination clinics and initiatives. This may include active participation in the delivery of vaccinations or the oversight of staff during clinics.
  • Achieves and maintains compliance according to organizational standards
  • Assist in identifying areas needing revision or new clinical guidelines.
  • Assists with the generation of new ideas, workflows, processes, and policies that improve quality, efficiency, and effectiveness within the program that are in support of departmental and organizational goals.
  • Participate in Quality Department special projects/programs.
  • Must be able to adhere to the Attendance policy and procedure.
  • Maintains positive rapport among co-workers.
  • Ability to collaborate, support and function in a teaching organization that builds coaches, and embraces interns and residency programs. Must demonstrate a passion for the team and consistently work with rotating doctors, residents, and interns, whilev continuously building a trusting, safe, patient centered workplace.
  • Demonstrate the ability to be culturally sensitive and respect diversity, works effectively and with individuals of different cultures and socioeconomic status, passion for service.

QUALIFICATIONS:

  • Education: Graduation from an accredited registered nursing program required. Bachelor's degree in Nursing preferred. Experience in lieu of a Bachelor's degree will be accepted.
  • Licensure/Certification: Current and unrestricted California Registered Nursing License, and current CPR certification required. Valid California driver license and active auto insurance required. As a requirement of this position, you must receive EPIC certification for the module you have been hired into.
  • Experience: Minimum of three (3) years of experience in healthcare preferred.
  • Essential Technica: Ability to utilize planning software, spreadsheets, and word processing software; input data, typing, copying, and effective use of the telephone.
  • Interpersonal Skills: Strong analytical, communication, interpersonal, rapport-building, presentation, and documentation skills (or apparent aptitude for such), and ability to plan and organize technical and analytical work. Ability to facilitate in a group setting and utilize teamwork skills.
  • Essential Mental Abilities: Strong planning, problem solving, documentation, and organizational skills. Ability to analyze and interpret data, analyze and interpret processes and needs; define problems and solutions. Able to manage time effectively and plan and implement objectives.
  • Work Eligibility: Must be legally authorized to work in the United States on a full-time basis. Must not now or in the future require sponsorship for employment visas.
