At VNA Health Group, we're transforming healthcare by empowering nurses to deliver compassionate care that helps patients live with dignity and comfort.
As a Per Diem Staff Nurse Hospice serving Monmouth County, you'll assess patient needs, manage symptoms, create individualized care plans, and support families through the end-of-life journey.
Responsibilities
How You'll Move Hospice Care Forward:
Complete a two-week orientation focused on hospice care practices, EMR systems, and safety protocols.
Assess patients with advanced illnesses and create individualized hospice care plans.
Provide education, emotional support, and guidance to patients and families as they navigate the end-of-life journey.
Collaborate closely with VNA clinicians and external providers to ensure seamless, compassionate care.
Document patient status updates and adjust care plans as needs and goals evolve.
Qualifications
RN with Bachelor's degree in Nursing
Certification in Hospice Palliative Care Nurse and Critical Care and Critical Care skills preferred
1+ year of supervised clinical experience as a registered nurse
Excellent communication skills
Strong Computer skills
Excellent organizational and interpersonal skills
Working Conditions/Physical Demand: Working indoors/outdoors with clinical equipment and use of phone and computer; car and valid NJ Driver's license
