Location: Suffolk County, Long Island | Per Visit / Field-Based | Non-Profit Healthcare

At Visiting Nurse Service and Hospice of Suffolk (VNSHS), our hospice team provides compassionate, patient-centered care that honors dignity, comfort, and quality of life. As a Hospice Registered Nurse (Per Visit), you'll play a vital role in supporting patients and families during one of life's most meaningful moments.
Your Impact:
The Hospice Registered Nurse (Per Visit) provides skilled nursing services and case management oversight, when assigned, to home hospice and Hospice House patients. This role collaborates closely with the interdisciplinary team to address physical, emotional, and spiritual needs while advocating for comfort and quality of life.
You Will:
Provide skilled hospice nursing care in patients' homes and the Hospice House setting.
Adhere to infection control practices and all VNSHS policies and procedures.
Apply the nursing process to assess physical, psychosocial, and environmental needs.
Collaborate with physicians, Nurse Practitioners, and hospice medical staff to maintain orders and manage changes in condition.
Demonstrate knowledge of the Hospice Medicare Benefit and end-of-life care principles.
Manage pain and symptoms using evidence-based end-of-life medications.
Advocate for patient comfort while supporting patients and families with kindness and compassion.
Participate as an active member of the Interdisciplinary Group (IDG), referring to team members as appropriate.
Maintain compliance with narcotic administration, distribution, and disposal requirements.
Provide case management oversight when assigned, including visit planning and prioritization.
Attend and actively participate in IDG meetings, discussing patient status, symptoms, and care plan effectiveness.
Complete accurate, timely documentation in accordance with Medicare guidelines.
Perform HHA supervisory visits as required.
Assist with orientation of new staff or students when needed.
Provide required federal and state patient notices.
Participate in weekend and holiday coverage based on agency needs.
Support the mission and values of VNSHS through professionalism, flexibility, and ongoing education.
What You Bring:
New York State RN license with current registration.
Ability to meet all NYS regulatory requirements for patient care.
Minimum of 1 year acute care experience.
Minimum of 3 months home health experience (other relevant experience may be considered).
Knowledge of hospice and end-of-life care preferred.
Strong communication, clinical judgment, and documentation skills.
Ability to work independently while collaborating effectively with an interdisciplinary team.
