Primary Work Location: Natalie's Place  

Natalie’s Place is one of several Cardea Health transitional housing programs for medically complex and behaviorally vulnerable unhoused individuals. Cardea Health’s transitional housing programs work with a variety of different referring partners across Alameda County. Our transitional housing programs provide supportive services including onsite nurse and caregiver supports, transportation to medical appointments, meals, housing support linkage, and more.   

Job Summary: 

Cardea Health seeks a highly motivated Registered Nurse with a special interest in working with people experiencing homelessness to work as a Nurse Supervisor. The ideal candidate will be invested in Cardea Health’s mission of serving marginalized populations with compassion and empathy. Cardea clinical staff need to be flexible, patient, and willing to work in challenging environments with a diverse population. Experience with homeless or incarcerated populations is highly desirable. 

Job Duties:

  • Supervise the orientation of new clinical staff members to the BHBH site, policies, and procedures. Evaluate the readiness of clinical staff to work independently and communicate concerns to clinical leadership 

  • Communicate clinical policy initiatives and changes to clinical team. Ensure that trainings, policy updates, and important information points are communicated across clinical staff 

  • Identify continuing education needs for clinical staff. Collaborate with clinical leadership to develop and execute continuing education programs 

  • Monitor staff performance, report training and coaching as indicated. Report issues with staff performance to the program director 

  • Direct the daily clinical care provided on site by Registered Nurses, Licensed Vocational Nurses, and caregivers 

  • Develop and enact quality improvement and quality assurance programs appropriate for clinical programs 

  • Arrange and attend case conferences with collaborating partners for complex clients 

  • Standardize clinical documentation practices according to site policies and scope of care 

  • Ensure communication regarding active resident issues occurs across shifts 

  • Coordinate personal care needs with the Caregiver Supervisor and caregiving team 

  • Assist in the arrangement and facilitation of on-site care provided by outside agencies such as home health, wound care, PT/OT and IHSS workers 

  • Provide quality care in accordance with Cardea Health policies, procedures, and standards. Ensure that all clients are treated with dignity and respect 

  • Conduct client intake assessments upon arrival to the program. Identify clinical needs and individualized goals of stay. Execute care plans for admitted clients in collaboration with primary, behavioral, and specialty care teams, medical directors, and clients 

  • Provide clients with appropriate information and education to make informed choices related to medical care and treatment that reflect individual preferences 

  • Maintain communication among other members of the clinical team. Communicate effectively to partner hospitals, community health clinics, homeless service providers, and other collaborating agencies 

  • Work with team members to develop creative solutions to meet the needs of individual clients. 

  • Participate in the collection and maintenance of data on program operations for reporting and analytical purposes 

  • Ensure adherence to infection control standards on site among staff and residents, including measures designed to stop the spread of Covid-19 

  • Ensure that clinical documentation is current, timely, and accurate 

  • Maintain confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA) 

  • Recognize and respond to medical emergencies, follow up with Medical Director communication or 911 activation 

  • Practice harm reduction principles, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect 

  • Other duties as assigned 

Minimum Required Skills & Qualifications:  

  • Associate or bachelor’s degree in nursing 

  • Hold current license as a Registered Nurse through the California Board of Registered Nursing 

  • Current Basic Life Support Certification 

  • Must have personal, reliable transportation to and from work sites and shifts 

  • May require evening, holiday, and weekend hours 

Preferred Skills & Qualifications: 

  • Self-directed and able to work with minimal supervision 

  • Detailed oriented and highly organized 

  • Proficiency in Microsoft Outlook, Word and Excel is required 

  • Prior experience with homeless or incarcerated populations is desired 

  • Two years of experience as a Registered Nurse 

Physical Requirements:  

  • Occasional sitting or stationary work 

  • Frequent standing, walking, kneeling, stooping, and ascending stairs 

  • Occasional lifting and carrying up to 20 lbs 

  • Frequent pushing and pulling 

  • Frequent reaching high, low, and level 

Work Environment:  

  • In-person onsite at a supportive housing facility. Constant work around and/or with people OR Interaction with patients, clients, and external customers
