Position Summary

Cleveland Health Group is seeking an experienced and highly personable Nurse Practitioner (NP), Registered Nurse (RN), or Licensed Practical Nurse (LPN) to join our growing team. The ideal candidate will have a passion for wellness medicine, excellent bedside manner, and a strong commitment to delivering outstanding patient experiences.

This role involves supporting patients through medical weight loss programs, hormone replacement therapy (HRT), wellness treatments, and ongoing care coordination while ensuring a luxury-level customer service experience.

Key Responsibilities

  • Conduct patient assessments and collect medical histories
  • Assist in development and implementation of individualized weight loss treatment plans
  • Support providers in administering and managing Hormone Replacement Therapy (HRT) protocols
  • Educate patients on treatment plans, medications, lifestyle modifications, and wellness strategies
  • Perform injections, phlebotomy, vitals, and routine clinical procedures
  • Monitor patient progress and document outcomes accurately in EMR
  • Coordinate follow-up appointments and patient communication
  • Deliver exceptional concierge-style customer service at every patient interaction
  • Maintain compliance with all clinical, safety, and HIPAA regulations
  • Collaborate with medical providers and administrative staff to ensure seamless patient care

Qualifications

  • Active Ohio Nursing License (RN, LPN, or NP)
  • Previous experience in weight loss, hormone therapy, functional medicine, med spa, or wellness clinic preferred
  • Strong customer service / hospitality mindset (REQUIRED)
  • Excellent communication and interpersonal skills
  • Comfortable educating patients on wellness and preventative health protocols
