GENERAL SUMMARY:
The Registered Professional Nurse uses critical thinking skills, professional judgement and leadership to anticipate, plan, direct and evaluate the care of the patients provided by a team of ancillary personnel and serves as a resource in a specific specialty area.

The Intake Coordinator is a Registered Nurse on the Behavioral Health Team who provides screening, case disposition and referrals for incoming telephone calls, walk-in inquiries about service and inquiries from within Insight Hospital and Medical Center. The position requires competent, independent decision making as well as collaboration with other disciplines and outside agencies to determine and obtain the most appropriate level of psychiatric care.

The Intake Coordinator administratively reports to the Director of the Behavioral Health and executes responsibilities under the supervision of the Department of Psychiatry Chairperson. In performance of all duties, the Registered Nurse-Intake Coordinator complies with the Illinois Mental Health amp; Developmental Disabilities Code, hospital policies, procedures and by-laws and reflects the philosophy, mission and values of the institution.
These duties are to be performed in a highly confidential manner, following the mission, values, and behaviors of Insight Hospital and Medical Center. Employees are further expected to provide a high quality of care, service, and kindness toward all patients, staff, physicians, volunteers, and guests.
Duties and Responsibilities:
60 % of Time Spent
Screening, Assessment and Referral: Screens incoming telephone calls to identify service request, urgency of need, patient resources and appropriateness of Insight Behavioral Health as a provider and level of care required. Refers to other levels of care in Behavioral Health Services when appropriate.
Provides resources, contact information and active linkage, when possible, for needed services not provided by Insight Behavioral Health Services.
Coordinates with partnering facilities to screen clinical charts with complex medical information, and assesses patient appropriateness for transfer into hospital
Nursing Care:
30% of Time Spent
  • Uses the nursing process, critical thinking skills, professional judgment, and the attributes of Careful Nursing to provide care for an assigned population of patients, assesses patient needs to determine care requirements.
  • Plans care and delegates tasks to ancillary personnel based on patient assessment
  • Demonstrates critical thinking and problem-solving skills independently.
  • Synthesizes information and resources to care for complex, higher acuity patients.
  • Completes designated unit specific competencies.
  • Completes at least 16 hours of continuing education, 30% in addition to mandatory in-service requirements, within the last year that is specific to daily clinical practice.
  • Participates in interdisciplinary rounds and facilitates discharge planning and patient education.
  • Assures high levels of quality of care through compliance
  • with regulatory standards, reference to current research related to patient care outcome and participation in the data collection and analysis process of ongoing quality monitoring.
  • Assists with admission and discharges to maintain the quality operations of inpatient behavioral health.

JOB SPECIFICATIONS
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge:
  • Current RN License in the State of Illinois required
  • Two years of behavioral health experience preferred.
  • Crisis Intervention training required within 90 Days of Hire -BLS Certified
  • Minimum of one year of clinical experience working on an inpatient behavioral health unit and/or related service line
  • -Knowledge of major mental disorders across the age continuum and levels of care.
  • -Understanding of the complexities of
  • Understanding of developmental norms across the age continuum and ability to apply age-appropriate expectations when engaging patients/families to obtain relevant history and current level of functioning.
  • Knowledge of substance abuse and dependency disorders across the age continuum and levels of care.

Skills:
  • Functional status screening skills to select the appropriate intensity of care or level of intervention required.
  • Competence in the use of the electronic medical record to document and retrieve pertinent patient information and generate required reports.
  • Excellent interpersonal skills necessary to engage patients and families in the screening process and to develop collegial relationships with professionals within and outside of the organization.
  • Communication skills to clearly and concisely transmit the essential referral information to clinicians regarding intake requests.
  1. Abilities:
  • Ability to prioritize work assignments, organize and track intake flow, meet turnaround time for call return and submit required reports.
