Description
Schedule: Exempt, minimum 40 hours per week | 7 days on/7 days off on call rotation with weekend hours

What You'll Do In This Role:
Start your nursing career at Huron Valley PACE! As an Extended Care Nurse at Huron Valley PACE, you'll provide essential after-hours and weekend care, ensuring participants receive timely, compassionate support when they need it most. Working a unique 7-on/7-off schedule, you'll take nursing and intake/referral calls, assess patient needs, and coordinate both scheduled and as-needed visits-helping manage complex conditions and prevent unnecessary hospitalizations. In close collaboration with the interdisciplinary team, you'll use your clinical expertise to assess, plan, and deliver care that enhances independence, promotes autonomy, and supports the overall well-being of the older adults we serve.
Key Responsibilities
  • Provide skilled nursing services in accordance with established care plans and clinical direction
  • Conduct participant assessments (phone and in-person) to evaluate needs and address concerns
  • Perform and coordinate nursing procedures to support participant safety and overall well-being
  • Monitor participant conditions and communicate changes to the clinical team
  • Educate and support participants and families on care needs and procedures
  • Triage participant calls and respond appropriately to urgent and non-urgent needs
  • Maintain accurate, timely documentation in compliance with organizational standards
  • Collaborate with the interdisciplinary team to support care planning and service delivery
  • Follow all organizational policies, procedures, and regulatory requirements
  • Participate in quality improvement initiatives and ongoing team collaboration

Skills, Knowledge & Expertise
  • Degree in nursing required
  • Must be licensed to practice as a registered Nurse (RN) in the State of Michigan.
  • Must have a minimum of 1 year of nursing experience.
  • Prior Home Health or Hospice experience preferred.
  • Strong verbal and written communication skills.
